Investigation process: what and why
Investigations following an adverse clinical outcome can be a difficult and emotional process for those involved. It is natural to find the process challenging, and it is important to seek support when needed. To help you understand what to expect, we have provided a document that explains the investigation process and what it may involve: NHS Maternity Investigation Process.
Within each Trust, the Maternity Governance Team is responsible for overseeing the investigation and management of maternity incidents in line with local and national guidance. They play an important role in ensuring that incidents are reviewed appropriately, lessons are identified, and opportunities to improve care and safety are recognised. Investigations will be conducted in accordance with the NHS Patient Safety Incident Response Framework (PSIRF) which aims to improve the response to patient safety incidents. The purpose of PSIRF is not to assign blame, but to understand what happened, identify opportunities for learning, and consider whether anything could have been done differently to improve the outcome or prevent similar incidents in the future.
